About HeartCare at Home

HeartCare at Home is a home care support programme for heart failure patients, helping you to feel safe and stay out of hospital.

It is a research driven care programme aiming to show that remote monitoring can improve quality of life for heart failure patients, reduce GP and hospital visits in Ireland, in the same way that other studies have found internationally.

We are researching all the benefits and outcomes for patients, the health service, and the wider community. By participating in HeartCare at Home, you will be contributing to a growing understanding and knowledge of the impact and potential of remote healthcare.

Meet the team

DONAL BAILEY
DONAL BAILEYDirector of Clinical Innovation
Dr. Bailey is a graduate of Trinity College Dublin since 2009. He is a General Practitioner and has further specialist training in Sports and Exercise Medicine through Queen Mary University London. During his training, he has worked in many areas including Emergency Medicine, Endocrinology, Rehabilitation Medicine, Paediatrics, Women’s Health, and Psychiatry. He joined the practice in 2016 and has a special interest in Sports Medicine and Preventive Medicine.
AOIFE COUGHLAN PhD
AOIFE COUGHLAN PhDProgramme Manager
Graduating from the University of Ulster in 2009, Aoife initially trained as a dietitian before completing a PhD in paediatric nutrition. With over 10 years’ experience in clinical research, she has held several clinical research roles at both Ours Lady’s Hospital Crumlin and Temple Street Children’s Hospital. Prior to joining Centric, she was the Programme Manager for Rare Diseases at Genuity Science, a programme providing access to whole genome sequencing to families with rare and ultra-rare conditions.  As the programme manager for HeartCare at Home, Aoife is responsible for the management and delivery of all research and clinical objectives.
CÍARA MATHEWS
CÍARA MATHEWSClinical Lead Nurse
Ciara Mathews is the Clinical Lead Nurse for the HeartCare at Home programme. Ciara qualified in general nursing in 2007. She has extensive clinical experience in cardiology and emergency nursing and has worked in both Ireland and Canada. Ciara joined the team in April 2021 and leads the clinical team in monitoring and providing remote clinical care to patients. An avid Crossfitter and busy mum of two, Ciara is dedicated to developing and improving the HeartCare at Home programme and believes remote monitoring is key to providing optimal heart failure care.
OLGA LEWIS
OLGA LEWISRemote Monitoring Nurse
With over thirty-two years nursing experience having trained in the UK in 1989, Olga returned home to Ireland in 2002 to continue developing her career in nursing. Olga has a wide range of nursing experience including Emergency Medicine and General Practice. After 18 years as a GP nurse, Olga changed direction and joined the HeartCare at Home team. She is extremely proud to be working on this new exciting venture.
CIARA MOONEY
CIARA MOONEYRemote Monitoring Nurse
Ciara Mooney lives in Co. Kilkenny and is a nurse with Heart Care at Home. Graduating in 2015 from Trinity College she has since worked in many of the acute hospitals around Ireland, learning and growing with experience. Although she has worked in many specialities over the years, her passion lies in Cardiology. Becoming a nurse was one of the most challenging and rewarding aspects of her life but she is proud to be able to provide such a high level of care to people with heart failure through the HeartCare at Home programme.
FIONA FOGARTY
FIONA FOGARTYResearch Coordinator
Fiona Fogarty is the Research Coordinator for the HeartCare at Home programme. Fiona completed her studies in Maynooth University and graduated from Biomedical Science in 2020. Having spent time in both research and diagnostic labs in Ireland, she brings a range of experience to the team. Fiona joined the team in January 2022 assisting in data collection and management. She is passionate about improving the lives of others, spearheading our educational campaigns on social media, and is excited about future developments of remote monitoring in healthcare.
