We already have over 100 patients enrolled in this study, here’s what they have to say:

It’s nice to know that you are there to keep an eye on us

I myself have had the first time since having CF, by having a feeling of more sense of security than ever before
The staff are fantastic, and my mam really enjoys their phone calls. It reassuring to know they will be checking in over the phone
‘If there’s a problem it is acted on immediately, I myself feel much more safer…and with excellent back up. Thanks

This is a new care package especially for heart failure patients:

Remote supervision by qualified medical staff

Fast & easy access to your personal medical team

Simple, health monitoring mobile app

This service is brand new to Ireland, but similar programmes have had great results throughout the UK & Europe for many years.

How does it work?

Patient's hands using Heartcare at Home mobile app

Our easy-to-use app makes it simple to submit your heart rate, blood pressure, weight and symptoms.

Nurse logging patient details into a computer

We check your data several times a week and if it’s stable, we continue monitoring in the background.

Nurse holding patient's hands

If we see changes in your data, one of our nurses will contact you within 24 hours to get you back on track.

Nurse listening to patient on the phone

We keep your GP/Cardiologist in the loop so that you avoid lengthy waiting times for appointments.

Why is this Free of Charge?

This service is part of a research driven care programme to show that remote monitoring can improve quality of life for heart failure patients in Ireland, specifically by reducing the number of GP and hospital visits. The study is being privately funded by Centric Health and Roche Diagnostics.
